Vaccines for School Entry Update & Medical Exemption Form

Image:

The passage of SB 277 eliminates personal belief exemptions for school and child care entry vaccines in CA. The state maintains an updated list of Frequently Asked Questions regarding SB 277. To qualify for a medical exemption, one or more vaccines must be considered by a physician, in his or her professional judgment, to be NOT SAFE for the child—nothing short of that qualifies under CA law. AAP has developed a medical exemption form to support CA AAP Chapter pediatrician members in record-keeping and in your ability to most easily provide schools and child care with the information required by the state for a child to qualify for a medical exemption from one or more school/child care-entry mandated vaccines under California law. The form is for use by physicians. It is NOT issued or required by the state or any school. The form is entirely optional; a physician may document a medical exemption on his or her letterhead, with the same information, if desired. AAP Chapter 1 Members who would like to receive a .pdf of the form via email please contact Info@aapca1.org.
